I acknowledge that I (or my child) am enrolled in the dance program offered by Miss Lucy’s School of Dance. I understand that participation in this program involves physical activity that may include, but is not limited to, strength training, cardiovascular exercise, flexibility work, and various forms of dance movement. I confirm that I (or my child) am in good physical health and have no known medical conditions or physical limitations that would prevent safe participation in this program. By choosing to participate, I voluntarily assume all risks associated with the physical activities involved. I hereby release Miss Lucy’s School of Dance, along with its owners, staff, instructors, and representatives, from any and all liability, claims, or demands arising from my (or my child’s) participation in the program. I understand that injuries may occur as a result of participation, including but not limited to: muscle strains or tears, sprains, fractures, shin splints, back or knee injuries, foot injuries, heat-related illnesses, or in rare cases, more serious medical issues including heart complications or death. By signing below, I acknowledge and accept full responsibility for any injuries or conditions that may arise and waive any right to hold Miss Lucy’s School of Dance or its agents liable now or in the future.
