Liability Release & Assumption of Risk
I acknowledge that I (or my child) am enrolled in the dance program offered by Miss Lucy’s School of Dance. I understand that participation in this program involves physical activity that may include, but is not limited to, strength training, cardiovascular exercise, flexibility work, and various forms of dance movement. I confirm that I (or my child) am in good physical health and have no known medical conditions or physical limitations that would prevent safe participation in this program. By choosing to participate, I voluntarily assume all risks associated with the physical activities involved. I hereby release Miss Lucy’s School of Dance, along with its owners, staff, instructors, and representatives, from any and all liability, claims, or demands arising from my (or my child’s) participation in the program. I understand that injuries may occur as a result of participation, including but not limited to: muscle strains or tears, sprains, fractures, shin splints, back or knee injuries, foot injuries, heat-related illnesses, or in rare cases, more serious medical issues including heart complications or death. By signing below, I acknowledge and accept full responsibility for any injuries or conditions that may arise and waive any right to hold Miss Lucy’s School of Dance or its agents liable now or in the future.
Medical Emergency
In the event of an emergency, I authorize Miss Lucy’s School of Dance and its staff to obtain emergency medical treatment for my child (or myself) if I cannot be reached. I understand that I am financially responsible for any medical treatment provided.
Photo / Video Release
I give permission for Miss Lucy’s School of Dance to photograph and/or video my child (or myself) during classes, rehearsals, and studio events. I understand that these images may be used for promotional purposes including (but not limited to) social media, website content, flyers, and studio advertising. I waive any rights to compensation for the use of these images.

I understand and agree that parents may take photos and videos of their own child during class and because of this I understand my dancer may be included in someone's picture. As a result, I acknowledge that my dancer may appear in other parent's photos and videos. I acknowledge that the studio is not responsible for any photos or videos taken by parents, including how they are used, shared, or distributed, and that the studio cannot monitor or control this activity. I agree to be mindful of other dancers’ privacy when taking or sharing photos or videos, especially if other children may appear in the image I acknowledge and accept this policy as a condition of participation.

​I understand and agree that the studio uses security cameras in both the waiting area and the studio space for the safety and well-being of all clients and staff.
Picture Day 2026 Photo Release
I give permission for my dancer to be photographed at Picture Day by Victor Jacek Photography and for all resulting images to be included in my digital photo gallery.
I understand that images from Picture Day may be used by Miss Lucy’s School of Dance and Victor Jacek Photography for promotional and marketing purposes, including social media, website, portfolio use, and studio marketing materials. I waive any right to compensation for the use of these images.
For Class Photos (Groups) - I understand that participation in the Class Photo (group picture of the whole class) is optional. By participating, I acknowledge that my dancer will appear in Class Photos and these group photos may be used for promotional and marketing purposes by Miss Lucy’s School of Dance and Victor Jacek Photography. I understand that Class Photos are shared as captured and are not edited, covered, blurred, or altered. I also understand that once class images are released, the studio cannot control how they are shared or used by others. If I do not consent to my child appearing in the Class Photo and its related marketing materials and sharing usage, I may choose not to participate in the Class Photo.
For Individual Photos and Tiny Ballerina Portraits - I understand that families who prefer not to have their child’s Individual Photos or Tiny Ballerina Portraits used for promotional and marketing purposes may opt out of commercial usage. This choice does not affect participation in Picture Day or access to my private digital gallery. To opt-out, simply fill out the Picture Day Opt-Out Form on Picture Day. These forms will be available at the desk on Picture Day. The opt-out form is only valid for Picture Day only and applies to Individual Photos and Tiny Ballerina Portraits (not group photos). If no opt-out form is submitted, permission is automatically granted.
